Community CareAge Foundation

Registered as a non-profit voluntary organisation in 2009, Community CareAge Foundation set up CareAge Nurse Stations and CareAge Health Centres in the community, providing free preventive primary health care services to over 3,000 singletons and / or low-income elderly people residing in public housing estates or grassroot districts. The primary objective of the CareAge Nurse Station is to provide preventive primary healthcare services to underprivileged residents aged 60 or above, with a view to enhancing their self-care ability, helping them to maintain their physical and cognitive well-being, reduce the number of visits to doctors’ clinics and hospitals, so they may truly age in place successfully.


We provide health education to improve their self-care ability, provide health assessment to identiachieve correct drug complaince, take care of themselves without medical advice, train them to be more self-caring and reduce the number of trips to clinics and hospitals, so that they can live a healthy and self-caring life in their familiar community. All services for the elderly are free of charge.

Mission

Provide free health knowledge and nursing consultation to the elderly, their families, and caregivers, strengthen preventive measures in the community, actively promote community members to care for and take care of the elderly around them, and establish a community of mutual support and care. Actively care for the elderly to train and improve the self-care ability of the elderly Promote public education, so that the public understands various diseases of the elderly, so that they can face the golden years calmly

Recruit registered nurses and registered social workers to set up "Yinling Nursing Stations" in public housing estates to provide comprehensive nursing services. Currently, 11 public housing estates have been set up in 4 districts of Hong Kong: Kwun Tong, Kowloon City, Sham Shui Po and Hong Kong Island East. nurse station

In addition to nurse services, free specialist consultation services (including ophthalmology, geriatric psychiatry, Chinese medicine, etc.) have been added to strengthen disease prevention in the community. Detect hidden diseases for the elderly and refer them to the appropriate public medical system as soon as possible. Currently available in Sham Shui Po and Ho Man Tin In the public housing estates, 2 Yinling medical stations were set up
