Message from the Chair

Dr. Chan Wah Fat



I am extremely honoured to be entrusted with Chairmanship of the Board of Directors of Community CareAge Foundation (CCAF). I am grateful to all the founding members and board members for their selfless contributions and leadership, which have enabled the Foundation to make big strides ahead steadily, and effectively over the years.


CCAF started with the “CareAge Nurse Station”, set up to serve community elderly since 2009, and now the Foundation has also successfully set up two “CareAge Health Centres”. The birth of the "CareAge Health Centre" is credited to the trust and support of the donors, the medical and health care professionals, volunteers, and the elderly. Together, the two operations elucidate the CCAF Board members’ commitment in the “Aging In Place” initiative. The Foundation will continue to learn, grow, and lead the needy elderly to meet the challenges presented by old age, and the increasing prevalence of chronic diseases. 


We are pleased to see the launching of the "Primary Health Care Blueprint" by the HKSAR Government in an attempt to strengthen the primary health care system in Hong Kong and set its development direction. We fully support and agree with this direction and hope that we can actively contribute to the Government's commitment. I believe that “Community nurse clinic” plays an important role to enable the elders age in place successfully in the comforts of their home. "CareAge Health Centre" and “CareAge Nurse Station” are there to bridge the service gaps by providing prompt consultations by nurses and medical specialists, to relieve the pressure of waiting time for specialist outpatient services.

In the development of primary healthcare services for the elderly, there are certainly difficulties and challenges to be faced, and the community will have to share more and more responsibilities. Reforming primary healthcare services is not an easy task, but I believe that with faith, optimistic and positive attitude, willingness to face the hardships, and enough courage to face any changes, the future is certainly promising!
