About "Care for the Elderly"


Founded in 2009, the Community CareAge Foundation (CCAF) is a non-profit charitable organization recognized by the Hong Kong government (Inland Revenue Department File No.: 91/10167), dedicated to providing intimate health consultation and nursing services for the grassroots elderly in the community. The founding and management team is mainly composed of professional medical elites. Nearly half of the board members have medical and nursing professional qualifications, including geriatric specialists, psychiatrists, registered nurses, clinical psychologists, registered dietitians, and registered social workers etc., providing professional medical knowledge, advice and supervision to the front line.


CCAF introduced the concept of community nurse clinics, hired registered nurses and registered social workers to set up "silver nurse stations" in public housing estates, and provided free health consultation services to the elderly aged 60 or above from low-income families. The services of the Yinling Nursing Station include public health education, physical and cognitive testing, and chronic disease management services. Family and community harmony. We hope that the beneficiary elders can improve their living habits, choose a healthy diet, prevent disease from worsening, and age at home with healthy body and mind and self-care ability.


In 2021, we set up our first CareAge Health Centre. In addition to providing a full range of nurse services, free consultation services by specialists (including ophthalmology, geriatric psychiatry, Chinese medicine, etc.) was launched to strengthen disease prevention in the community and detect hidden diseases for the elderly as soon as possible. Refer to the appropriate medical system for medical treatment as soon as possible to achieve the effect of home aging.


CCAF is a member organization of the Community Chest of Hong Kong and a member of the Hong Kong Council of Social Services. Our operation is supported by donations from the society (private, charitable foundations, industrial and commercial organizations, etc.). CCAF's services create and generate positive social impacts. In 2019, it was awarded the "2019 Most Effective Small and Medium-Sized Non-Government Funded Social Service Organization Award" by the Hong Kong Council of Social Service.

Vision

To bring a healthy and happy old age life to the elderly and improve the quality of life of the elderly. Elderly at home expects the elderly to live healthily, independently and safely in a community environment they are familiar with. Neighbors help each other and build a harmonious community.

Mission

Provide free health knowledge and nursing consultation to the elderly, their families, and caregivers, strengthen preventive measures in the community, actively promote community members to care for and take care of the elderly around them, and establish a community of mutual support and care.
