The heavy demand of public medical services makes stable patients with chronic diseases queue a long time for follow-up and treatment. Therefore, CCAF comes up with the idea of CareAge Health Centre as a solution to this situation. With combination of medical and care services, grassroot elders are first screened by our registered nurses. Then they will be referred to the Health Centres to consult doctors if necessary. This model helps the elders to receive timely support during the queueing period of public services. At the same time, our register nurses follow up with their physical and mental health status and the treatment outcomes, and provide professional consultation services to the elders and their caregivers. 


In 2021, we established our first CareAge Health Centre in Sham Shui Po. Not only offering holistic care by nurses, we also engage different specialists (including ophthalmology, psychiatry, orthopedics and Chinese medicine) to offer free clinic services. The Health Centre supports the elders living in the community with enhanced disease prevention in the community, early identification of health problems, as well as timely referrals. The second CareAge Health Centre in Oi Man Estate in Ho Man Tin was opened in March 2022. We keep our promise with the expansion of our free-of-charge healthcare services to the grassroot elders and their caregivers, and support them to live healthily and stay safe at home and in the community that they familiar with.
